  1. Contact Information [to be added directly to our vendor list]
  2. Certificate of Insurance [proof of insurance]
    1. Must be submitted to the corporate office from the vendors insurance agent/broker [email to: office@vertusproperties.com]
    2. "Vertus Properties, Inc." must be the certificate holder for certificate to be valid
    3. General Liability for up to $1,000,000 per occurrence
    4. Workers Compensation Insurance
    5. Auto coverage [if applicable]
  3. Completed W-9 form 
  4. Copies of Licenses [if industry standard]

**Vendors will not be paid prior to the completion of the 4 required items**
